Complete the affidavit accurately
MVR-46F is a notarized request to cancel the record showing the vehicle in your name. It certifies that the title was assigned and delivered, so do not use it to substitute for an unfinished handover.
Keep the submission record
Follow the form’s instructions for mailing or submitting at a license plate agency. Save a copy with your sale and plate-return records.
