Replacing a lost title: HSMV 82101

Use the missing-title guide first to distinguish a lost paper certificate from an electronic title or one held by a lender. Download the current duplicate-title application from FLHSMV when that process applies.

Reporting the sale: HSMV 82050

The Notice of Sale and Bill of Sale form serves different purposes depending on the sections completed. Use the Notice of Sale guide for filing and recordkeeping; a receipt alone does not resolve every ownership situation.

Authorizing a representative: HSMV 82053

Salvage and derelict documents

HSMV 82363 and HSMV 82137 concern different salvage or derelict situations. Start with the salvage guide and establish eligibility with the appropriate recipient or office before completing an application.

Bring the supporting records

Have identification, the existing ownership document, lender-release evidence if relevant and the agreed buyer information ready. Ask the processing office what originals, signatures and additional evidence it requires for your circumstances.
